Illness or Absence

Arrangements in case of illness or absence.

It is really important that we can always quickly contact parents or carers if Children are taken ill or have an accident at School.

With this in mind will you please fill in the CONTACT FORM  giving details of telephone numbers (Please try to give a landline if possible) and any relevant medical information when you admit your Child to Reignhead.

 PLEASE REMEMBER TO UPDATE THIS INFORMATION AS SOON AS DETAILS CHANGE. 

This form is our emergency link with you.

In the case of Absence due to illness or medical/dental appointment, a telephone call, followed by a note on your Child's return to School is essential.

If absences are for longer periods please keep us informed so that our Education Welfare Officer is fully aware of the situation, as we now have to monitor and log reasons for absence.
